When moving out of a secondary residence and not moving into a new secondary residence on national territory, you must deregister within two weeks of moving out.
If other family members (spouse, minor children) are moving as well, you may use this form if you have been living in the same household and will continue to do so in the future. If this is not the case, we kindly ask all family members concerned to complete their own individual forms.
